Your audiology report must state the type of hearing loss and severity. It
must be signed by your audiologist, and include his/her scope of practice.
If you do not have a copy of your most recent audiological evaluation, you
may provide signed letter from your audiologist stating your type of hearing
loss and severity. This letter must be on your audiologist’s business
letterhead.

If you have additional disabilities, please provide a letter from your
doctor stating your diagnosis and its severity. Allergies, physical
limitations, restrictions and special accommodations needed related to job
equirements are all appropriate to mention. This letter must be signed and
on your physician’s business letterhead.
