Dear Parents of Deaf and Hard of Hearing
\nChildren\/Youths (ages 5 to 18),<\/p>\n
My research team at the University of Washington is studying the
\nquality of life of children and youth who are deaf or hard of hearing
\nD\/HH). The goal of our study is to create a new questionnaire that
\nmeasures quality of life specifically for children and youth who are
\nD\/HH. Currently, there is no existing questionnaire that looks at
\nquality of life for children and youth who are D\/HH. We hope that this
\nnew measure will help researchers in a variety of fields (health,
\naudiology, education, social work, and public policy) and ultimately
\nhelp children and youth who are D\/HH. In order to create this new
\nquestionnaire with standardization and normative data on deaf and hard
\nof hearing children and youth, we need your help!<\/p>\n
We need more participants!!<\/p>\n
Each eligible youth participant (ages 11 to 18) will be paid $25 for
\ncompleting the youth questionnaire. Parent participants will be paid
\n$25 for completing the parent questionnaire for each eligible child
\n(ages 5 to 10).<\/p>\n

\nand look in the left yellow column for the custom ID (platypus2009).
\nThen click on the link(s) below that to complete the screening form.
\nYou must fill out separate form for each child. This takes less than 5
\nminutes to complete.<\/p>\n
If your child is eligible, a research staff will mail you a packet of
\nforms to sign and return. After we get the forms, a research staff
\nwill give you a password to begin filling out the survey form online.
\nFor parents of children ages 5 to 10, the survey will take less than
\n30 minutes. For youths ages 11 to 18, it will take about 30 to 45
\nminutes. If you or the youth prefer ASL administration, we can mail
\nyou a DVD. The English print is available online and in paper form if
\nyou prefer this.<\/p>\n
