\nCase Manager <\/p>\n
Methodist Mission Home, a 110 year old non-profit agency serving adults with disabilities seeks an experienced Case Manager for a vocational rehabilitation program. Fifty percent of the residents on our 25 acre campus are deaf and many are challenged by multiple disabilities. SCHI teaches adults with disabilities the vocational and life skills they need to achieve their personal best. Within 12 to 24 months, residents are ready to transition into their own apartment and work full-time. <\/p>\n
Must have at least a Bachelors Degree preferably in a discipline related to rehabilitation and\/ or residential care and experience working in social service . Has strong written, oral and manual communication skills and can exercise good decision making skills. Establish and maintain effective, professional rapport with sponsoring agencies and serve as the case expert for each assigned resident. Assist in resident staffing including medication review with consulting psychiatrist. Assist in teaching Independent Living skills classes and related group sessions. <\/p>\n
