Invitation to participate in Youth Quality of Life-Deaf/HOH survey
online (will pay $25 for each participant)

We are excited to announce that the Quality of Life-Deaf/HOH
instrument is ready for standardization and norming. We need about 400
participants, both youths (11 to 18 yrs) and parents of children
(5-10) who are deaf or hard of hearing. This project is funded by the
National Institute of Health.

Each eligible youth participant will be paid $25. Parent participants
will be paid $25 for each child (age 5 to 10) who is eligible.

There are two parts to the survey participation process. First, you
will complete a screening form online (less than 5 minutes). If your
child is eligible, a research staff will mail you a packet of forms to
sign and return to us. After we get the forms, a research staff will
give you a password to begin filling out the survey form online. For
parents, the survey will take about 30 minutes to complete. For
youths, it will take about 45 minutes to complete. If you or the youth
prefer ASL administration, we can mail you a DVD. The English print is
available online and in paper form if you prefer this.

You must fill out separate form for each kid.
