The Texas Governor’s Committee on People with Disabilities invites you to take an online survey called, “H1N1, Texans with Disabilities Prepare for Pandemic Influenza.”
The online survey is available online in English and Spanish. Additionally, a Resource document with downloadable forms and important links is also available. Please forward the link to the online survey to Texans with Disabilities in your local communities and encourage them to take the survey. This survey is also available by mail, in large print and by telephone at 512-463-5739. The information results will be posted and available in November of 2009.
