Education: Graduated from an accredited college or university with a Bachelor’s degree with a major specialization in Human/Behavioral/Social Services or related field is required. Bilingual American Sign Language required.

Description: This position is located in the Intellectual Developmental Disabilities division in Integral Care. An HCS Service Coordinator will perform moderately complex local authority/service coordination work based from the Home and Community-Based program. The HCS Service Coordinator’s duties include but are not limited to leading the consumer’s team of natural supports and HCS providers to develop a person directed plan, traveling to consumer homes, Day Habs, or other places in the community on a monthly basis to monitor the appropriateness and progress of the HCS Services and goals on a Person Directed Plan, update the person directed plan annually or as needed, document every contact with the consumer and other collaterals, deal with situations where crisis prevention is necessary, coordinate meetings between service providers, meet with Guardian/LAR, and consumers monthly to review services in the HCS program, and follow up with telephone and e-mails with families and providers.

The HCS Service Coordinator is also responsible for State CARE data entries, linking consumers and families to resources when needed, and making sure that it is all done following policies and procedures. This position works under general supervision with moderate latitude for the use of initiative and independent judgment. Reasonably regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned.

Hours: 8:00AM – 5:00PM, Monday-Friday. May require a flexible work schedule.

Salary: $2,830 – $3,371 monthly, salary based on related education and Integral Care experience. Upon successfully passing our agency bilingual exam, an added annual bilingual pay up to $3,730 would apply.
