Needs assessment survey for stakeholders in the disability community

I am writing to invite all members of the disability community to participate in a short survey regarding the service needs of Texans with disabilities. The Department of Assistive and Rehabilitative Services (DARS) has partnered with the Child and Family Research Institute at The University of Texas at Austin’s School of Social Work to conduct a needs assessment of services for individuals with disabilities.

As part of the needs assessment, I would like to ask you to complete a survey regarding your experiences as a caregiver, advocate, professional and/or individual with a disability. Your responses will be used to identify existing service gaps.

The survey is anonymous and confidential. The survey is available in English, Spanish and Vietnamese. It should take about 10 minutes of your time.
